By creating a smaller patient base within the membership model, Dr. Valerius and Aubrey Spangler, APRN they are able to focus on each patient and become their partner for all of their health and wellness needs. This includes increased availability and access, and less wait time.
In return for your membership, you will be receiving many benefits, including:
- Increased availability for appointments
- Complete health perspective. Dr. Valerius and Aubrey Spangler, APRN, can work closely with you throughout any health issues, including following up with your specialists, discussing results from tests or diagnostic procedures, reviewing additional treatment options, and more.
- More convenient direct contact: Your online personal health portal gives you a secure place to send and receive messages to and from Dr. Valerius, Aubrey Spangler, APRN, and staff, including prescriptions, reminders, and lab results.
There is a minimum enrollment of 6 months if paying monthly, and 12 months if paying annually. If you decide the membership isn’t right for you after your minimum enrollment period, please provide the practice with a 30-day written notice to terminate your contract.
We offer two payment options: monthly or annually. You will not be charged until your membership begins. Membership fees are charged on the 1st of each month should you elect to pay monthly.
One of the great benefits of this membership is that it’s much easier to connect with Dr. Valerius or Aubrey Spangler, APRN- especially in the manner you prefer - in person, by phone, or via portal. Portal access is anytime, and a reply can be expected within 48 hours. The Personalized Primary Care Membership without Insurance includes four office visits per membership year.
We will have on-call coverage with another Practitioner in the rare case that both Dr. Valerius and Aubrey Spangler, APRN are out of the office so that if an urgent need arises, you will still get the care you need.
- First, you’ll see Dr. Valerius or Aubrey Spangler, APRN to discuss your health issue. If needed, you will be referred to a specialist.
- Dr. Valerius and Aubrey Spangler, APRN are able to remain actively involved in your health issue. They can follow up with the specialist, are available to discuss your treatment options, and can work with you to schedule follow-up visits, and/or coordinate discussions with the specialist.
- Having your personal provider, a trained clinician who knows your entire health situation in detail, at your side as an active advocate can be enormously comforting and helpful to you, as you make important medical decisions.

The membership fee is designed to be affordable for most people, and sustainable for the practice in the long-term. There is a possibility of increased rates at the start of a new year. The practice will notify patients in advance.
No! This is not a replacement for insurance. The membership fee is to sustain our practice model of fewer patients and more accessibility (communication and longer appointments). For our insured patients, the Membership fee was designed to cover the services not typically covered by insurance companies. For patients out-of-network, with Medicare benefits, or cost sharing plans, our fee was designed to provide you with the care you deserve. This allows us to support you in creating health rather than only to treat you when you are sick.
